What is naltrexone?

A non-addictive medication that quiets the reward loop.

Naltrexone is an FDA-approved opioid antagonist used for alcohol use disorder since 1994. It binds to the opioid receptors that alcohol activates — without alcohol's effects — so drinking feels less rewarding and cravings often decrease over time.

It is not:

  • A controlled substance — no withdrawal, no high, no diversion risk
  • A sedative — it won't affect your energy, mood, or ability to function
  • Disulfiram (Antabuse) — naltrexone won't make you sick if you drink
  • A lifelong commitment — many patients use it for 6–12 months and stop

What it is:

  • An oral pill taken daily, or as directed by your provider
  • Backed by 30+ years of clinical research
  • Effective for both reducing drinking and supporting abstinence
  • Generally well-tolerated; most side effects (mild nausea, headache) fade within 1–2 weeks

Do I have to stop drinking before starting naltrexone?

No. Naltrexone can be started while you're still drinking. Many patients reduce gradually as the medication takes effect. The exception: if you're physically dependent on alcohol (severe daily drinking, withdrawal symptoms), you may need medical detox first — your provider will assess this at your consult.

Will my insurance see this on my record?

If you use insurance, the visit will appear in your insurance claims like any medical visit. If privacy is a concern, the cash-pay option keeps everything off your insurance record. We follow HIPAA — your records are confidential either way.

How long until it works?

Most patients notice reduced cravings within 1–4 weeks. The "extinction" effect — where drinking feels less rewarding — typically builds over 2–3 months of consistent use. We track progress at each monthly visit and adjust as needed.

Are there side effects?

Common (usually mild and resolve within 1–2 weeks): nausea, headache, fatigue, dizziness. Less common: insomnia or vivid dreams. Rare but serious: liver concerns — we check liver function before prescribing and periodically after. Avoid naltrexone if you have severe liver disease or are taking opioid pain medications.

We try other evidence-based options. Acamprosate, disulfiram, and combination approaches with therapy are all on the table. The first medication isn't always the right one, and that's not a failure — it's part of finding what works for you.
