Blue Cross Blue Shield is a federation of independent local plans rather than a single insurer, so benefits differ meaningfully by state.
"BCBS" covers a family of independent licensees — Anthem, Highmark, CareFirst, Florida Blue, Premera, Regence and others — each operating in its own territory with its own benefit design and formulary.
That structure is exactly why verification is not a formality with BCBS plans. Two patients with cards that look nearly identical can have quite different behavioral-health cost-sharing depending on which licensee issued the plan and in which state.
